The town was named after Saint Theodore and was officially established as a municipality in 1957. Its coastal location has made it an important area for fishing and maritime activities throughout its history. The clear waters and white sand beaches have also made it an increasingly popular tourist destination in recent years.

The indigenous Mangyan people have long inhabited the area, and their cultural influence can still be seen in local traditions and crafts. Spanish missionaries arrived in the 17th century, establishing the first Catholic churches in the region. During the American colonial period, San Teodoro developed its agricultural sector, particularly in rice and coconut production.
